A vitally important book that highlights the sorry state of current UTI testing/treatment guidelines， with a supposed “gold standard” （mid-stream urine cultures） that relies on assumptions that not only were never validated， but have even been thoroughly dispelled since then。Namely， the test assumes that the healthy bladder is sterile， that growing a bacterial species in a urine culture therefore incriminates it， that growing several of them implies sample contamination， and that if there is a p A vitally important book that highlights the sorry state of current UTI testing/treatment guidelines， with a supposed “gold standard” （mid-stream urine cultures） that relies on assumptions that not only were never validated， but have even been thoroughly dispelled since then。Namely， the test assumes that the healthy bladder is sterile， that growing a bacterial species in a urine culture therefore incriminates it， that growing several of them implies sample contamination， and that if there is a pathogen， it will appear in the culture。 Exactly none of those assumptions are remotely true。And on the basis of such a flawed test， ignoring the base rate， numerous patients with relevant symptoms are told that they do not have an infection and are thus denied antibiotics and left to suffer。 Even those for which the culture finds something are prescribed needlessly broad-spectrum antibiotics， since the prescription is based on the resistance data for whatever bacteria the culture happened to pick up， which are not necessarily the causative pathogens。It is seriously time to revisit current practices and the assumptions upon which they rest。 Those assumptions have permeated the way that UTIs have been approached so far， with even most “known urinary pathogens” in truth only incriminated by association。 。。。more
